Admissions Policy

This a free program for low income families. We service expectant mothers and children from birth to 2 years of age within the 19122,19132 and 19133 zip codes. Applications must be completed in person. The parent/guardian will need proof of income, child's birth record, insurance card, shot record, and a copy of the child's last physical. Expectant mothers applying, will need proof of income, insurance card, and medical documentation verifying pregnancy.
